Job Summary
The Patient Care Coordinator plays a crucial role in supporting healthcare providers and interdisciplinary teams to enhance patient care. This position is focused on improving clinical outcomes and patient satisfaction while managing healthcare costs and ensuring timely and accurate communication with payors. Responsibilities include managing resource utilization, care facilitation, and discharge planning, as well as driving performance improvement initiatives. The Coordinator will oversee a designated patient caseload, facilitating self-management, providing family support, and optimizing resource use.
Minimum Qualifications
- Education: Graduate of an accredited nursing program required; Bachelor's degree in Nursing preferred, or graduate of an accredited Master's in Social Work program.
- Licenses/Certifications:
- Current Registered Nurse license in Texas or
- Current Master Social Worker license in Texas required, LCSW preferred.
- Certification in Case Management is required within two (2) years of appointment to the role.
Experience / Knowledge / Skills:
- Three (3) years of nursing or social work experience in an acute hospital setting preferred, or equivalent experience in relevant clinical environments.
- Experience in utilization management, case management, discharge planning, or similar quality/cost management roles is preferred.
- Outstanding interpersonal communication and negotiation skills.
- Demonstrated leadership abilities.
- Proficient analytical, data management, and computer skills.
- Working knowledge of discharge planning, utilization management, and case management.
- Comprehensive understanding of pre-acute and post-acute care venues, community resources, and transitional care processes.
- Strong organizational and time management abilities, capable of managing multiple tasks efficiently.
- Ability to work independently while exercising sound judgment with various stakeholders.
- Effective verbal and written communication skills.
Principal Accountabilities
- Coordinate patient care progression throughout the healthcare continuum.
- Maintain active collaboration and communication with healthcare teams to ensure timely and appropriate care.
- Proactively identify and resolve obstacles to discharge effectively.
- Collaborate with physicians and team members to facilitate care and monitor patient progress.
- Complete and review necessary diagnostics, treatment plans, and discharge planning in a timely manner.
- Manage documentation accurately within patient records.
- Assist in utilization management and quality screening for assigned patients.
- Identify at-risk patient populations using approved tools and procedures.
- Facilitate effective discharge planning and ensure communication with patients and families regarding care continuity.
- Conduct assessments and develop individualized care plans in collaboration with healthcare providers.
- Participate in performance improvement activities and assist in the collection of financial indicators related to patient care.
- Assist in mentoring new hires and support their professional growth.
- Ensure adherence to safety protocols and quality standards in delivering care.
- Exemplify commitment to providing personalized patient experiences in alignment with Memorial Hermann's service standards.
- Other duties as assigned.
